Is penile filler safe?
No medical procedure is free of risk, and penile filler is no exception. Hyaluronic acid is widely used as a dermal filler in the face and is broken down gradually by the body. That makes it a more forgiving material than older, non-absorbable fillers or unknown substances, but it can still cause problems.
It is also important to know that, in most countries including the UK, the EU and the US, dermal fillers are not specifically approved for use in the penis. Penile use is off-label, and evidence on long-term outcomes is limited. The risks are lower when the treatment is done by a doctor, in a sterile setting, with a recognised hyaluronic acid product and careful technique.
Before treatment, tell your doctor about any bleeding disorder, blood thinners, allergies, autoimmune conditions, previous penile treatments or injections, and any recent genital infection. These can change whether filler is suitable for you.
What is normal afterwards
These effects are common and usually settle on their own:
- Swelling, which is often greatest in the first two or three days and can make the penis look larger than the final result
- Bruising at injection points, fading over one to two weeks
- Tenderness or a feeling of tightness
- Temporary changes in sensation while swelling settles
- Slight firmness or small areas that feel uneven early on, which often soften as the filler settles
Possible complications
Risks you should know about. Complications are not common, but they do happen. They include lumps or nodules; an uneven, irregular contour; filler migrating away from where it was placed; infection; delayed inflammatory reactions; and, rarely, damage to blood vessels affecting the skin. Results also vary, and some men are unhappy with the degree of change. Your doctor will discuss your individual risks before you decide.
Nodules and irregularity
Filler can gather into lumps or leave an uneven surface, especially after vigorous activity too early or with larger volumes. Early irregularity can often be improved by massage. Persistent lumps may be treated by dissolving the filler with hyaluronidase.
Migration
The skin of the penis is loose and very mobile, so filler can shift over time, for example towards the base or towards the head of the penis. This can change the shape. Following the aftercare advice, particularly avoiding pressure and sexual activity in the first week, helps reduce this risk.
Infection
Any injection can introduce bacteria. Signs include increasing redness, heat, pain, swelling or discharge, and sometimes fever. Infection needs prompt medical treatment, usually with antibiotics.
Delayed reactions
Occasionally, firm, tender swellings appear weeks or months after treatment, sometimes after an infection such as flu elsewhere in the body. These are treatable, but need assessment by a doctor.
Blood vessel problems
Rarely, filler can compress or block a small blood vessel, affecting the skin supply. Warning signs are unusual pain, pale, dusky or blotchy skin, or colour changes that spread. This is an urgent situation.

Recovery timeline

Aftercare
- Keep the area clean and dry, and wash your hands before touching it
- Avoid sex and masturbation for about a week, or as long as your doctor advises
- Avoid tight clothing, cycling, swimming pools, saunas and hot baths for the first few days
- Avoid strenuous exercise for a few days
- Only massage the area if your doctor has shown you how
- Use simple pain relief if needed, as advised by your doctor
- Attend your follow-up check, in person or online

Dissolving the filler
One advantage of hyaluronic acid is that it can usually be dissolved with an enzyme called hyaluronidase. This may be used to treat lumps, irregularity or migration, or if you decide you no longer want the result. Dissolving may take more than one session, and hyaluronidase itself can occasionally cause swelling or an allergic reaction.
Flying home
Because filler is an outpatient procedure without general anaesthesia, many men can travel within a few days. Your doctor will confirm when it is appropriate for you and arrange online follow-up after you return home.
