About ZSI
Zephyr Surgical Implants (ZSI) is a Swiss manufacturer of penile implants. Its devices are less widely used than those of Coloplast and Boston Scientific, but they offer designs that can help in particular anatomies.
As with every brand we use, the choice is made with you after examination. Other options include the Coloplast Titan, the AMS 700 and Rigicon devices.
ZSI 475: two-piece inflatable
Most inflatable implants have three parts, with a fluid reservoir placed behind the abdominal wall. The ZSI 475 is a two-piece device: the pump and reservoir are combined in a single unit placed in the scrotum, connected to the two cylinders in the penis.
- No reservoir in the abdomen, which can be helpful after certain abdominal or pelvic operations, such as some bladder, bowel or transplant surgery, or when placing a reservoir would be difficult
- The penis becomes soft when deflated and firm when inflated
- Because it holds less fluid than a three-piece system, the flaccid and erect states may feel different from a three-piece device; your surgeon will explain what to expect
ZSI 100: malleable
The ZSI 100 is a malleable (semi-rigid) implant. Bendable rods are placed in the penis; there is no pump. You position the penis up for intercourse and down afterwards.
- Simple to use, with no pump to operate
- Fewer mechanical parts and usually a shorter operation
- The penis always stays firm, so it is less discreet

Who may suit a ZSI implant?
A ZSI device may be considered when a standard three-piece implant is not ideal: for example, if placing an abdominal reservoir would be risky, in some revision operations, or when a man prefers a simpler device. It is not automatically the right option, and your surgeon will explain the trade-offs.

Surgery and recovery
The operation is done under general or spinal anaesthesia through a small incision, usually in the scrotum. You stay in hospital for up to two nights and in Istanbul for about six days. Intercourse is usually possible after 4–6 weeks, once your surgeon confirms healing. See surgery in Istanbul and recovery week by week.
Risks
Before you decide. Risks include infection, bleeding or haematoma, mechanical failure over time (for inflatable devices), erosion, and changes in how the penis looks or feels. An implant is a permanent change: if it is removed, other treatments for erectile dysfunction are unlikely to work. Results vary from person to person.
